The Journey to Adulthood: Transition Planning in Idaho

Transitioning from child to adult

Families whose child is almost 18 face unique challenges when thinking about their youth transitioning into an adult. Often, youth with disabilities have the need for additional training, supports and services outside of those offered in the educational setting. For families this can be a confusing time that can be difficult to navigate. It is best to start thinking about transition for a child by the ages of 15-17 years old.

In Idaho, schools must work to help students with disabilities plan for graduation and life after high school. This is called transition planning. The school staff, case manager, vocational rehabilitation counselor, the family and the student all are involved in this planning.

There are also a number of agencies that can help with transition planning.
