Helps a family identify resources and services needed in order to maintain stabilization and support of their child. This may include, but is not limited to, renegotiation of their subsidy payments, access to grant funding for their child, or finding a qualified provider in their area.

Helps a family nearing crisis by providing expedient referral and coordination of services and providers in an effort to stabilize and safely keep the child/youth in home or support to route the child/youth to residential care.

You are eligible even if your family no longer lives in Idaho. Our goal is to ensure long-term stability and provide families with the tools they need to thrive.

We offer a comprehensive range of supports, including:

  • Annual Reviews: Support in ensuring that our families are in compliance with the requirements and have everything necessary to secure ongoing access to support in the year to come.
  • Assessment & Referral: Quick assessments to identify your family’s specific needs and connect you with community resources.
  • Case Management: Intensive support for families with high needs or those nearing a crisis.
  • Financial Advocacy: Assistance with renegotiating subsidy payments and accessing adoption, family, or community grants and funding resources.

As children grow, their needs often change. RISE helps families navigate the process of determining if they are eligible to request a change in the adoption subsidy amount that was set as part of their original or subsequent adoption or guardianship agreement. If your child’s needs have increased and the associated costs, particularly those not covered by insurance and beyond typical child-rearing expenses, have become more significant, our case managers are here to help. They would be glad to work with you in exploring whether requesting a renegotiation may be a suitable option for your situation and support you in submitting a formal request to the Idaho Department of Health and Welfare.

Yes, our team of Post-Permanency support workers is available to provide immediate and intensive access to service through resourcing, care coordination, and problem-solving during a crisis. Our staff is trained to assess a situation quickly and link our families to the appropriate level of support, whether that involves intensive in-home services, advocacy within the juvenile justice system, or coordinating residential care if necessary to keep the child safe.

The Post Permanency Grant Program ended in 2024 and was replaced by the revamped Thriving Children’s Fund. This fund is intended to support Post Permanency families in managing costs related to parenting a child with special needs who was adopted or placed in a legal guardianship.

To be eligible for funds, the child must:

  • Be under the age of 18
  • Have been adopted* in any state or placed in a legal guardianship through the child welfare system in any state (*cannot have been adopted by their stepparent or birth parent)
  • Have a current need related to their emotional or behavioral health, physical health, education or development, culture, trauma history, interests, or relationships with permanent or birth family

Is there a limit to the amount of funds? The amount of money available for Thriving Children Post Permanency Funds is limited and changes each year. To make sure as many families as possible can get help:

  • Priority may be given to families who have not received funds before
  • Requests approved one year may not be approved the next year
  • Requests that go over $5,000 for a child over one or more years need additional approval
  • Funds may be awarded as requested, in part, or denied

What can funds be used for? Services, activities or supports related to the child’s needs which are:

  • One time or time-limited
  • Not covered under other resources, programs or community organizations such as monthly subsidy payments, Medicaid, insurance, sponsorships, scholarships, etc.
  • May include time-limited assistance in the event of financial crisis threatening the stability of the permanent placement.

Examples of services, activities, or supports that can be paid for:

  • Transportation to maintain birth family relationships
  • Parent registration, travel fees, and child care for training to meet the child’s unique needs
  • Extra-curricular activities for the benefit of the child that are unaffordable to the family
  • Time-limited in-home or community-based therapies (additional requirements apply)

Examples of services, activities, or supports that cannot be paid for:

  • Out of home placement including residential treatment services
  • Orthodontics
  • Attorney/court fees
  • Emotional support animals
  • Private school, daycare, or child care
  • Reimbursement for services, activities, or supports already purchased
