At RISE Services, Inc., we offer Targeted Service Coordination for any individual eligible to receive developmental disability services. The Targeted Service Coordinator (TSC) will create an Individual Support Plan (ISP) along with the participant and their support team which will link to services that will meet their needs.

The TSC will advocate and coordinate services by helping gain access to, monitor, and maintain services with the intent of being as independent as possible.

Examples of support would include but are not limited to residential, developmental therapy, mental health, transportation, social and recreational opportunities, employment, and medical services.

Yes. One of the main jobs of a Targeted Service Coordinator is to connect you with the right people and programs. This includes finding specialized supports, new day programs, and much more. They can also help you find “natural supports” in your community, such as local support groups, housing assistance, or transportation options. Your coordinator stays in touch with you regularly to make sure these resources are actually meeting your needs and helping you stay independent.
