Independent Living with the Freedom of Choice
At RISE Services, Inc., we help make dreams of independence come true. Our Supported Living program assists adults with developmental disabilities in developing and maintaining independent living skills in their own homes. Each person has the choice of where to live, who to live with, where to work, and how to spend their time.
Our Supported Living staff offers a high level of personal care services within each person’s home. These services are tailored to meet individual needs while emphasizing increased independence and participation in the community. Staff encourages independence in tasks for which the individual has the skill and/or ability. Individuals are supported in living arrangements which are typical of those in which persons without disabilities reside.
RISE offers hourly and 24-hour services to support the needs of each person. Supported Living Services is a Medicaid reimbursed program.

Supported Living is designed for adults who want to live in their own home or apartment rather than in a facility. Unlike a traditional residential group home, which is a shared space managed by an agency, Supported Living at RISE offers a more independent approach where individuals can have a lease in their own name.
There is choice involved in the living arrangement, but it can vary depending on each situation. For individuals who already have a home or have identified preferred roommates, RISE can build supports around that setup. In other cases, RISE also offers homes where participants can sign a lease; however, roommate options are typically based on current participants and availability at the time.
Our role is to bring support directly to you. Staff assist with daily tasks based on each person’s unique needs, helping individuals maintain independence while ensuring they have the support necessary to be successful.
Our team provides “wraparound” support tailored to your life. This includes helping with:
- Household Management: Shopping for groceries, planning healthy meals, and keeping your home clean.
- Health & Safety: Managing medications, getting to doctor appointments, and responding to emergencies.
- Financial Skills: Creating a budget, paying bills on time, and managing your personal spending money.
- Community Connection: Getting involved in local hobbies, visiting friends, and using public transportation.
Yes. One of the biggest benefits of Supported Living is that we can provide up to 24-hour care depending on your authorized service plan. Even if you do not need someone with you every minute, we ensure that help is always available if an emergency arises. This gives both the individual and their family members peace of mind, knowing that professional support is always just a phone call away.
